the fact that you're seeking assistance through any means you can and reaching out here helps. That being said, go to any hospital, tell them you do not have any insurance and that you *are* manicly depressed and you *will* receive treatment. Ask them if you can apply for state aid retroactively for the help you require.
As far as transportation goes, start walking towards the hospital, when you see someone who may be able to help you, ask them for a ride. If no one helps you keep walking until you either arrive or you find help.
Trust me on this one, you definately have the strength for it, you just have to learn to "flex" it a little bit this time to get where you're going. Drag, push, borrow or steal... you'll get where you're going

I'm sorry that you are feeling so depressed. It's not fun, I know. Have you tried calling a crisis center? The people there are very understanding and can give you some good information. Go for a walk and get some exercise. Exercise is great for improving your mental health. Seriously. And remember this will not last forever. Things change. Start planning for how you can help change your life for the better. Best wishes!

Call any of the crisis lines in your area. They will help you, and they will get you in to see a therapist who can bot work with you and get you the medication you need. They can even help get you to the office. Hang in there. There's people to help.
If you don't know what the number to the crisis line is, call police dispatch or emergency, or even the emergency room at the hospital and they can provide the number.

Turn to god, that always helps me when Im down and out. And if you are not a god fearing person then turn to something that makes you feel good. Take a walk and speak to people on the street or go to the public library and pick up a good book...Good Luck... Life is short, enjoy it...

Grocery store. Pretend to shop and meet people. If you get lucky you can take advantage of free samples.
