I was actually just watching a documentary about Casanova on the History Channel. It's a real person. He was someone where men would pay to have them do the husband duties while the husbands were busy. Kind of like a personal assistant. He fell in love with one woman, but then in the middle of his spree of woman contracted an STD. She didn't want anything to do with him of course after that.

It was pretty interesting.

Casanova was an actual person, who actual was considered to be a player in the old days, and that is why the expression is used.
