You would use the 1040 long form. Schedule A is where the itemized deductions are placed. In order for them to matter they must be greater than the standard deduction which for a married couple filing jointly is $10,300. You can also include medical expenses, property taxes, interest on your home mortgage, state income taxes besides gifts to charities.

To deduct contributions you have to itemize instead of taking the standard deduction. You use form 1040 schedule A to itemize, and would only do it if your total eligible deductions are larger than the standard for your filing status.
