If you need an actual photo copy you would need to complete form 4506 and send that along with a check for 39.00 to the service center where you filed, it will take 60 days to process your request

Your first option would be to contact your employer. If you cannot get duplicates from your employer, you can contact the IRS and they can provide you with them. However, W-2's through the IRS are only available the year after the year the W-2's are filed with the IRS, so for your 2004 W-2, they are filed with the IRS in 2005, so they would be available from the IRS in 2006.
If you are due a refund for 2003, you only have until April 17, 2007 to file your return and get the refund. If you file after that, the government takes it and your refund is gone forever. You only have 3 years to file for a refund.

Sending out W-2s wouldn't be NEARLY as much of a hassle for this employer as explaining to the IRS why they think it's too much of a hassle to do. If you don't hear anything in another week or two, contact the IRS again and talk to them about how you can file without a W-2. If you have your last paystub, they should be able to help you through this.

As of last year, a person could go down to their local IRS office, and request a print out of everything from one year back. There is no charge for this. It will only contain Federal Tax Info, no State.
Also, remember that you can only expect to get a refund from 3 years back only. If you had extenuating circumstances which kept you from filing you could sometimes get an exception.

Contact all your past employers and ask them to issue your W-2 for wages. The Government do not issue them. . .you send the W-2 to the Government with your tax forms.
Once you have all your past W-2, go to some tax preparing agency and file back taxes.

YOU HAVEN'T FILED SINCE 2001??? Even if you are getting a refund you MUST file every year!!! You really need to get in touch with someone at the irs to find out what you need to do. I hate to say it but since you didn't file, you may have to pay penalties!
