If you've got a UK postcode, go to the Royal Mail site here: http://www.royalmail.com/portal/rm/addressfinder?pageId=pcaf_a_search
Just type in the postcode and it will give you a list of the possible addresses to go with the postcode.

If you don't know the house number as well as the postcode then this can be difficult but if you know the persons name you might be able to look them up on http://www.thephonebook.bt.com/publisha.content/en/find/residential/residential_numbers.publisha
Use this to look up all possible addresses covered by the post code.

If you put the correct name, street name, town and post code then the local postman usually manages to get it to the correct home in the street! (Don't forget to put your details on the back of the envelope, as sender, in case the item has to be returned because the person has moved.)

If you are looking for a UK address, you can use the Royal Mail website. It may not give the exact address, but it will give you the correct side of the street, or maybe the houing development.
