Unless the rules have changed recently, then of course you do. It is best to know where you will be staying before you apply for a visa. It is a very good idea to keep a friend or family member updated on your travel plans. Although China has become more travel friendly, there are still a lot of sticky situations that foreigners can easily find themselves in, especially if you are not traveling in the regular tourist areas. And make sure of how your travel insurance works. I have had many coworkers and friends who have had minor food poisoning from restaurants there.

For most people, yes, you do.

However, if you are holding Japanese, Singapore passports etc you don't need any visa to stay up to 15 days when you enter China from Shanghai airport. and there are many exceptions
