You absolutely need to find out why you have had chronic back pain for so long. You may have injured yourself, you may have arthritic changes, you may be suffering severe muscle spasms. Your doctor should then be able to tell you how to relieve the pain. The doctor can recommend at home remedies, physical therapy, medications like muscle relaxants and refer you for chronic pain management.
Just by what you say, it sounds like you may have severe back spasms. Resting your back when you can, relearning how to use your back and heat should help release the muscles.
One of the problems with muscle spasms is that they can squeeze nerves which will cause miserable pain. They can also press nerves up against the spine which can cause pain. Then you get into a cycle of pain - it hurts, you tense, when you tense, it hurts and so on.
My source is me. I broke my back body surfing last July. I have compression fractures at the T-6, T-12 and L-1 (that's mid back and lower back). This causes pain in the areas above and below the fractures. So my entire back pretty much hurts all the time. Some is muscular in nature, some is nerve damage. I also have degenerative arthritis and osteoporosis so my back is healing but very slowly. I am allergic to most pain medications, so once my back was stable I was referred to physical therapy and a pain management clinic. I am learning how to support my back using other muscles and how to use my back in a healthier way. I also take Baclofen which is a muscle relaxant. My pain has gone from a constant 9/10 to a manageable 5/6.
Back injuries are nothing to mess around with. You use your back for virtually everything you do.
Good luck.

This may sound strange - Go to a Foot Doctor to have inserts made for your shoes. . . . I didn't think it would work but over 6 months ALL my back pain disappeared. In increments they gradually build up the inserts & it straightens out what was messed up. Good luck . . .

Try aleve/naproxen sodium. Heat and ice compress (15 min on/ 15 off/ etc). Walking is great for back pain. If it continues or worsens, see your doctor. Be weary of heavy duty pain killers (narcotics). They tend to mask pain and are highly addictive.

Lie on your stomach and place an ice pack (a bag of frozen vegetables will work just fine) on the part of your back that hurts most. There has to be *at least* one layer of cloth between the ice pack and your skin. Leave the ice on for 20 - 30 minutes. The ice will help.

You may need to go to the chiropractor and get an adjustment. You should also look online to find some back stretches. Muscle relaxers would help, if its the muscles in your back.
