Lung cancer is one of the most fatal cancers worldwide, causing up to three million deaths annually. Only one in ten patients diagnosed with this disease will survive the next five years.
This fatal disease like all other cancers, results from abnormality in the body's basic unit of life, the cell. Normally, the body maintains a system checks and balances on cell growth so that cells divide to produce new cells only when needed. Interruption in the balance of this system causes an uncontrolled division of cells that eventually forms a mass known as a tumor. Any of the tissues in the lung can become cancer, but most commonly, the cancer comes from the lining of the bronchi. Since this disease tends to spread very early in its course, it is very life-threatening and one of the most difficult cancers to treat. Although it can spread to any organ in the body, the adrenal glands, liver, brain, and bone, are the most common metastases.
the greatest impact on risk of lung cancer is long-term exposure to inhaled carcinogens. Most common of which is exposure to tobacco smoke. The occurrence of lung cancer is strongly linked with cigarette smoking, with about 90% of the disease arising as a result of tobacco used. The numbers of cigarettes smoke over time increases the risk. Among those who smoke two or more packs of cigarettes per day, one in seven will die of lung cancer. Passive smoking or inhalation of tobacco smoke from other smokers sharing or working quarters, is also an established risk factor for the development of the disease. Non-smokers who reside with a smoker have 24% increase risk for developing lung cancer compared with non-smokers

See Mathew P's answer above.
I'm posting to correct some wrong info from others. Smoking absolutely increases your risk for lung cancer (and many other cancers). It also increases the right for many noncancerous medical problems. See the link below under sources. Key to remember is that individuals do not define a correlation, but rather a large population of individuals defines the correlation.
Also, heart failure caused by the low oxygen from the lung cancer is not the leading cause of death in lung cancer patients.

Usually, lung cancer isn't diagnosed until it's too far ahead to make chemo/radiation/surgery a possibility, thus death usually comes sooner for victms of lung cancer. My mom is the one exception that I know of. She had a cold for a couple weeks and wanted to go to e.r. for a chest x-ray,she thought she had pnemonia. She did, and was treated. Later, the radiologist sent her a letter stating he saw an abnormality and to contact her g.p.to follow up. She did,she had 1st stage lung cancer. She had chemo/radiation, no surgery. She smoked through the entire thing, never stopping, always saying it could have been caused by anything, that it might not be her 3pack a day habit. No, that wasn't the problem. You know what, she survives till this day, still smoking her 3packs a day.

There are few cases of full remission from stage 4 cancers, although they are getting more common every year that goes by as therapies improve. So stage 4 isn't always a sure death sentence but most often it is. I had a family member recently die from colon cancer, he lived for almost 5 years in stage 4 as they did different life extending therapies and surgeries. I have another family member with stage 4 breast cancer who is in her 3rd year living in stage 4. Both were very young for their respective cancers so I think youth and being fit played a big factor in living allot longer in stage 4.

All cancers, if left untreated (and sometimes even treated ones) are fatal. I imagine cancer in lungs is particularly delicate, as there isn't much spare tissue that can be (comparatively) easily removed, such as breast tissue or skin.

Believe it or not but most people who die of lung cancer actually die of heart failure. Because of the cancer the lungs can't function properly and starve the heart of oxygen, causing it to fail.

Lung cancer causes your lung cells to be replaced with cancer cells, which do not have air sacs (alveoli). Your lungs stop absorbing oxygen.
It also spreads to other organs (metastasis) and causes the liver, bones, brain, etc., to stop their normal function.
