The only things the Supreme Court requires is letting the suspect know that he has the right to remain silent, that anything he says can be used against him, that he has the right to have an attorney present when being questioned, and if he cannot afford an attorney then one will be provided to him for free. Then the person reading the rights must confirm that the suspect understands the rights.
The Supreme Court has specifically ruled that there are no exact words, phrases, or sentences that must be used.
Some jurisdictions, like the FBI, add in something like, "If you choose to answer my questions, you may stop at anytime and request an attorney." This is completely unnecessary.
