He doesn't seem to care if he lives or dies, eats or not. Everything he does do is erratic. He'll scrape a plate a thousand times. He touches things with just one finger, even petting a dog.

Mabye a stroke of some kind, sounds more like depression.
See if he has any of these other symptoms:

Sounds like OCD.. Ritualistic behaviors are one of the biggest signs (scraping a plate a certain amount of times) But there are other things like depression or even schizophrenia (not eating, poor personal hygiene) so many mental disorders mimic these symptoms only a doctor can say for sure.
