They'll probably just give you the shots right there at the crossing. Don't worry.

No- you don't need any immunization cards. I crossed this border a couple of times. Once with an expired tourist visa. I expected to have to pay a fine, but the Ecuadorian official just stamped my passport and told me to move along.

When you cross the border you will be mobbed by dozens of people offering to change money, give you a lift to whatever city you're going to, etc... Don't even bother with any of these people. The money is probably fake and the prices they will offer are ridiculously high. Read your guidebook carefully and talk to others who have made the crossing to figure out how to do it "by the book". Do what you can to find a bus that goes all the way to your destination in Peru, rather than dropping you off at the border- that will save you a headache. These busses DO exist and if you ask enough questions you'll find out which ones they are and were they are stationed.

I crossed that border by bus twice, and I never needed it either time. I don't remember ever being asked for it in fact. If it was not trouble to get one, would not be a real bad idea, however I would not fret over it either. That border can be a bit of a mess... If you have it it would save trouble maybe, should they ask for it. Hope this helps. Just a note, if you can get to cross on the Ormeno bus line, they kind of help you across, it is a very good bus line to cross with. VERY comfortable
