The first, the DA 4856 is the counseling form utilized to document any counseling session between a soldier and a subordinate. Counseling sessions can take on multiple forms. You can have a positive counseling, where you want to annotate what a good job a soldier has been doing. You can have a negative counseling statement, where you are professionally letting a soldier know of their short-comings and what they need to do to improve their performance. There is also a general information counseling, also known as an in-briefing, when a new soldier joins your section and you take the time to let them know what there job responsibilities and duties will be, along with what is expected of them from the unit/section. All of these are filed for future reference and destroyed once the soldier leaves that section/unit.

The second, the DA 2166-8, is actually a NonCommissioned Officer Evaluation Report. This is a very descriptive form utilized in evaluation of the job performance of and NCO in the Army. This breaks down job performance into separate categories with a section available for bullet comments by the rater. These forms are placed with the NCO's permanent record and stay with them for their tenure with the military.
