It seems that now (5 years later) his PSI levels are way up. Does anyone know of someone who has experienced this? If so, where did the cancer spread if any and what treatment did they use? Any articles, personal experience or related information would be very helpful. Thanks!

I suggest your father have his testicles removed ASAP. Your situation is deemed to turmoil. Having his balls removed the cancer will spread no further. These are your two options, His life, or his balls....this may be a thinker.

PSA, not PSI. Recurrence following prostate surgery happens perhaps 15-20% of the time. It means he still has cancer, and it must be treated. It needs prompt professional attention, as a recurrence will spread faster than the original cancer if untreated.

The first question is how extensive the cancer is. They will probably do a bone scan to see if it's metastasized into his skeletal system. They may do an ultrasound or MRI of the prostate bed as well.

It's possible that radiation treatment of the prostate bed may be appropriate follow up, maybe not. The alternative is hormone therapy: Unless the cancer is very advanced, it requires testosterone to grow. By reducing or eliminating his production of testosterone, the cancer may be stopped or slowed. As the idiot above mentions, oriechtomy (removal of the testicles) is one very radical approach to accomplishing this. More likely, they will put him on a combination of drugs designed to mimic castration. The effect of the drugs is reversible, unlike the operation. Diet is also important: limit meats, eat tomato products, brocolli, nuts. No refined sugars.
