it all depends on what type of guy he is. You could go with the weird boxers thing or what i would do, his favorite candy, attached to a mix of all his fav songs mixed with a little of yours.

At your age a card would be the right thing believe it or not I still have my 1st card given to me back in 1978 by my 1st girlfriend and I often look at it and wonder what she is doing and where she is at this time and that stays with a fellow more than jewelry or clothes, or even a anything materialistic, take my advice boyfriends come and go but you only have one virtue so keep it

do something cute like a treasure hunt or something like that and get him something simple because you guys probably aren't planning on getting married anytime soon and i doubt he is going to be your only boyfriend if you are both about 13. just have fun with it
