My ears ocasionally block up, but behind the eardrum i get a fluid build up. It is really painful, and it reduces hearing. It feels like you are under water. Any ideas, the doctor wont give anything and says i have to let it clear by itself.

Hi, I'm an Audiologist.

If you feel confident that your Dr. is not treating your symptoms aggressively enough, you should go for a second opinion - preferably to an Ear specialist. And have a complete hearing evaluation that will include a test (called a tympanogram) to measure whether the plugged sensation is simply due to Eustachian tube dysfunction that causes negative pressure in the middle ear space, or whether there is actually fluid behind the eardrum.
