If you've decided that Catholicism is the specific sect you'd like to convert to (and to be officially on the books for it, not just believing in Catholicism), then look up a local Catholic church in your phone book's yellow pages. Contact that parish, and they can arrange a meeting with a priest to explain what you'll have to do (which for the most part just comes down to a baptism).
Keep in mind though that there are lots of different sects of Christianity, all trying to say their own way is the "right way", and will typically bash Catholicism whenever they get the chance. So be wary of some of the answers you get on this thread.

Get hold of a Bible and start reading it - one of the Gospels (Matthew, Mark, Luke and John) is a good place to start, because it's all about who Jesus was.
Find some Christians and talk to them about it. Ask if you can go to church with them.
Being a Christian is about having a relationship with God, through Jesus. Please don't think of it as a religion to convert to. It's not about rules and rituals.

Research churches in your area. Choose one you are comfortable with after visiting a few. Attend regularely to fellowship with other believers and learn about God. Read the Bible, fellowship with God Himself! Talk to the pastor- or priest- about how you can take the final step.

Recognize that Jesus came in the flesh and that He rose again after being crucified. Ask Jesus to be your personal savior. The price has already been paid. I would like to suggest you go to a good bible believing church.

You'd have to go ask leaders at a catholic church, but I'm pretty sure you just have to be baptized.
