My cat needs to gain a 3-5 lbs...he likes to eat but often has a hard time keeping food down. My thinking is that maybe there's some high-calorie, low-volume, treats out there which I can give him one per hour or something like that.

If your cat is underweight and having a hard time keeping food down then it is very sick and needs to see a vet now. You can try feeding it a high calorie but without meds the cat will only die. It could actually be harmful to the cat so take the cat to the vet and do as the vet says to help your cat.

There are some new cat treats out Greenies that are very good and healthy for cats that you could try giving him they help the cats digestive system to make sure that it is properly cleaned out so they can digest and eat properly. He could be having problems with hairballs. Try a hairball remedy medicine it comes in a tube and you can let him lick it off of a treat or a dish.

there is a high calorie nutritional supplement called Nutri-cal that is made specifically for this. Comes in a tube and is cheap (around $6-$7) It is usually sold by vets, or you may be able to get it online. Cats usually like it and you can give them small amounts throughout the day.

I agree with the first post, the cat is sick or maybe has a lot of worms, there is no such thing as "hard time keeping food down", if you were to have diahrrea every time you eat then you'd go to the doctor because there is something wrong with you. The same goes to the cat. Once he is cured whatever he eats will get him fat.
