you could give the wrong drug to the wrong patient at the wrong time with the wrong dosage strength through the wrong route...that could kill someone. Imagine you accidentally give a blood-pressure lowering medicine to someone with an already low blood pressure. Or Imagine you give an antibiotic to someone who is allergic to it. Even if you get the drug and the patient right, you could end up giving them a toxic dosage. Or if you give it through the wrong route (say trying to inject cough syrup) the drug might not be effective. Giving a drug at the wrong time can cause it to interact with other medications they're taking, or can decrease the effectiveness or cause uncomfortable side-effects.
