There is no difference. Ham Radio is usually considered more informal and Amateur Radio would be the formal term to use. The term "Ham" for amateur radio is more of a nickname and came about a long time ago from telegraph days when it meant "A poor (telegraph) operator"

There is no difference. Radio Hams are amateur radio enthusiasts who need a licence to broadcast.

"ham" is a colloquial term for "amateur".
