I would say I was flattered if I saw the giver of the compliment was only trying to butter me up.
Mostly if the compliment is meant you would say thank you, dismiss it or say that you were touched.

It's impossible to say without hearing their tone of voice or observing their body language to go along with the words.

If they said it and rolled their eyes for instance, It would probably be a good guess that they were being sarcastic!

If the giver were a big-name celebrity, or an expert in the field they are complimenting you on, you could be both flattered and honored. Anything can be turned into sarcasm if you want to make it so. Immodesty would be if you agree with the compliment, and state or imply that you deserve it.

It would be hard to say.
reading body language and hearing the tone in the voice.

personally i get a little worried with complements.
I don't know how to respond and i say thank you and try and ignore it happened.. because i just don't know how to receive.
I'm a giver - i have been trying to force myself to accept complements with out just giving a complement back.. In other words shifting the situation back onto them.

So with all those mixed feelings and emotions of me receiving a complement.. i don't know how anyone would be able to pick up if i was any of the above.

If you feel you want to complement someone.. Just do it.

I would say flattered if I was caught of guard or embarrassed. Just a fall back response when a person is caught off guard.

Most of the time I think they're being modest.
