She needs to see a specialist and get this diagnosed. If has even just a little fluid in her lungs they need to treat that agressively along with the pedal (feet) edema. Take her to a new do or insist that her regular doc treat this and find out what is going on.

The swelling, shortness of breath and wet lungs concern me even if they don't concern her doctor. She may have a "good pulse" in her feet but a dusky toe says something different. The toe may be a diabetic complication. I just don't know enough about her or what meds she's currently on. Still...the fluid overload bothers me. Although edema is associated with DM, the swelling in the feet and ankles, the fluid in the lungs, the shortness of breath - these are symptoms of CHF. I strongly suggest that she sees another doctor soon for a second opinion.
