Is it a felony or a misdemeanor if the secretary at the dr's office calls in a prescription for narcotics (pain pills, valium, sleeping pills...) to the pharmacy w/o the dr's knowledge? What are the penalties? What about going across the boarder to get them, are there consequences?

1) She'd get fired. She would lose all benefits except her vested amount in her 401k or similar.
2) She could get arrested. If a first time offence, a good attorney might get a plea bargain down to a misdemeanor. Otherwise it's a federal felony offense.
3) In any case, she would have humongous fine and legal bills, possibly jail time.
4) She would have great difficultygetting a job in the medical field ever again, and moderate difficulty getting any sort of a job again. Remember, more and more employers are doing background checks on their employees, especially those in responsible positions.
