I'm not sure what country you are travelling from?? The Indonesian Government website should list their import regulations. I would also get in touch with a pet moving company so that they can ensure that your cat meets all the requirements. The country you are leaving from will also have guidelines re animal exports, again should be available on your gov't's website.

Most airlines would have somewhat the same procedure.
For instance, Singapore Airlines's procedure is such that you must fax or email pet's details like breed, age, color, name, sex, weight, weight and size of cage and also the Agricultural & Veterinarian Authority (AVA) reference. If you have more than one pet, you have to ensure that they each have their own cage. Once authority to uplift has beem granted, you can just check-in your pets (in their individual cages) accordingly. Do note that some countries do not allow pets to be checked in through passenger terminal. Therefore they'd need to go through cargo terminal.
SQ charge a pet in a cage according to normal excess baggage charges.
