i got in a small accident in 2006, the insurance people sent me a form to fill out and send to DMV...i havent done that yet. its been several months, should i still send it?

YES you should send it in. How do you know the other person didn't send it in? You don't.

If the other person sent in the SR1 form, the DMV will cross check to see if you submitted one...if you didn't, they'll suspend your license.

You're only required to submit the SR1 form if the damage is $750 or more, to either vehicle.

Better be safe than sorry...just send the form in. The insurance company sending one to you was out of courtesy...they are under no obligation to do so. It's their way of helping you comply with state law.

Yes. Any accident causing personal injury or property damage in excess of $500 must be reported.

if the other party reported but you failed to do so your license will be suspended and then you have to get a restricted license for a probation period ( typically 6 months to 2 years )
