It could be the 1040EX, the 1040A or the 1040; depends on how old you are, how much you made, and whether you can be claimed on another person's return.
Call the IRS topic telephone line and listen to the disk for "What form to file." You might also find "Who should file." interesting.
If you are still at home with your parents, check their tax booklet they got in the mail for the information you are looking for. Its all in there.
