At my daughter's weddings we did send invites to everyone as we had to let caterer know the numbers. We did have a good idea on how many would be in attendance, though. Our caterer needed to know 4 days before the event so we sent invitations out 2-3 weeks ahead of the event. We did not include children unless they were a part of the bridal party.

Invitations are not extended for rehearsal dinners. Only those involved in the rehearsal and their significant others are expected to attend. And those involved who cannot attend need only let everyone else know the day before.
