i am allergic to silver, and if I leave it on for more than 12 hours, I break out. I broke out again recently, and I have been putting cortizone on it (what my dermotologist suggested) as I normally do, but it isn't helping. I still have itchy red bumps at the base of my neck. Is there anything else to apply?

My suggestion is not to wear silver anymore if that is causing your rash.

Don't keep using cortizones as they produce side effects. I know - I have been using them for years and they lose their effect.

Are you also sensitive to certain fabric that may be causing itchy red bumps around your neck? You may have contact dermatitis.
If so, it is best to abstain from using these type of fabric.

Sweat may also be another thing that may cause the itches. The important thing is to find out what triggers the reaction and take preventative measures.

There are two options :

1) Stop wearing silver. Allergies often start off as mild (just an itchy rash) but can develop into full-blown anaphylactic shock (Google it if you don't know what it is) at a later stage because contact with silver primes your body's defense mechanisms. The fact that cortisone is not working anymore is a sign that it is gettinhg gradually worse). A higher dose of cortisone will also work (temporarily), but you should look up the side effects of corticosteroids first - there are MANY and they're NASTY, and EVERYONE who uses them for a prolonged period WILL get side effects. Google "Cushing's disease" for a nice summary of the side effects (the disease is not caused by the steroids, but it has the same picture).

2) Use clear nail polish to cover the silver and protect your skin. If this doesn't work stop wearing silver. It's your life.

Did you try painting your jewelry with clear nail polish so the silver doesn't touch your skin? I have heard this works, but I have never tried it.

There is a liquid that you can buy that actually is meant to cover up the jewelry to prevent irritation from allergies. You can buy it through some jewelry stores, or I believe you can buy it through firemountaingems.com.
