Go here to find out requirements, per U.S. Department of State: http://travel.state.gov/travel/cis_pa_tw/cis/cis_1033.html
Go to the Belarus consular site to get details about visa and other requirements: http://www.belarusembassy.org/
Note: I see that Belarus has a scarcity of medical things like prescriptions and supplies. A kind gift for family members might be to bring some medical staples such as over-the-counter pain medications, allergy medicines, antiseptic, etc.
You might also visit the Thorn Tree Forum to talk with people who have been recently to Belarus to see what they recommend you bring with you (for yourself or for others), what to pack, what to expect, etc.

Your passport, and if you didn't retain citizenship in Belarus, a visa for Belarus.

Since your country (Belarus) does not allow dual citizenship, you have to apply for a visa in their embassy.
