im 22 healthy male(i think), i used to have a powerful memory till i was 15, when i was 15 i left to another country to go to college, ever since then the problem gets worse, i forgot my name under 3 circumstances
can anyone give any help or info or hints

You need to go see your doc. for a check-up. It would be difficult to diagnose your symptoms. You might need to go to a specialist.
It is a little unusual to have memory loss @ your age.
Have you had any head trauma?

wow that is serious. if you forget your own name then you may have some serious issues. i usually forget my best friends name or somethng but not my own. that could be a sign of a serious condition. you may need to see a neurologist to be certain there is not a tumor or lesion.
