there is a car rental called "maggiore" in the airport of naples capodichino. it's very serious, i travelled a lot with them.
by the way, if it's the first time you drive on the left, i'd suggest you not to practice in naples...it's like hell!
and even if you are on foot, take care!
however, in naples public services as train, bus, underground, don't work very well, so if you want to go around (especially not only in the city centre) the car is almost necessary.
