He has a hernia, and if not operated on it can become strangulated, which means the intestines are twisted and the stool can't move through them and they can rupture. Then he will wind up in the ER and have emergency surgery. That isn't the ideal way to have the hernia repaired. It is much simpler if he does it as elective surgery (he and his doctor pick the time and place) that way he has time to be prepared properly for the surgery. It is much easier to perform surgery on and empty gut that it is one that is full of stool and possibly infection from the ruptured intestine. Just tell him if his doctor has recommended surgery he needs to get it done. Doctors don't recommend or do surgery today unless they feel it is truely necessary. If he is truely concerned have him get another opinion. Most insurance companies will pay for the second opinion. Tell him good luck and God Bless.

In my nonprofessional opinion, this sounds like a hernia.
Easily corrected with surgery, but some men just live with it. Depends on how big the bulge is in the scrotum.
