This is a complicated question. There dozens of different fetishes and there is always somebody who likes them. To say it is wrong for consensual adults to decide what form of sexual activity they want limits everyone's freedom.
Back to the question, "does ballbusting hurt the guy". No, unless you pull out the sledge or are intently trying to hurt someone you won't. Hollywood fills our heads with the myth that the testicles are so sensitive that men will pass out or feel excruciating pain if hit in the genitals.
In reality the male genitals will hurt about as much as the female's counterpart when hit. A female's privates seem to be well protected from serious damage so I don't know how much power it would take to do serious damage. I do know that men's genitals will not even begin to sustain damage until 40 lbs (or the equivalent in force) is used.
The origins of ballbusting come from a man's desire for pressure on his genitals shortly before ejaculation. For unknown reasons this has been commonly known to intensify the pleasure in sex. Some neuropsychologists believe that the brain misinterprets pain for pleasure. Keep in mind that this is just our best guess rather than a scientific answer. Not all men experience intensified pleasure when their genitals are grabbed before ejaculation and few men like ballbusting outside of sex.
Bottom Line: if your boyfriend wants something done to him that you think is way to painful to have done to your own privates then don't. If you are afraid in anyway that a certain action could possibly do serious damage to him then again don't do it. Men are far more likely to be infertile because of genetics or because they have a laptop on their lap then from being kicked in the testicles. So just loosen up a bit.

Why would it be wrong? A fetish is something (normally very kinky) that a person is into... so if he is into it then he is wanting it.. some people are turned on by pain. During before and after sex!
