That could be interpreted two ways.

Calling a woman, MADAM, is a cordial and gallant way to address her.

Calling a woman A MADAM, means that she is the head of a prostitution ring.

Very different things.

A Madam is a prostitute who is in charge of a brothel (IE a Female Pimp)

When you call a woman Ma'am it is like refering to she as Mrs. which wouldn't be offensive to most women, although many women feel that calling them ma'am, makes them feel old, so they prefer to be called Miss.
