No matter why you are holding a door open or offering to hold a door open, the trick is to hold it so that you are not standing in the doorway. Stand along the side of the opened door itself, or stand a bit behind it and hold it open. This is something that kids do instinctively, but it's a conceptual problem for many adults.
Remember that chivalry itself isn't dead. But if your arms are full, why shouldn't a woman hold the door open for you? Why should an elderly man feel that he must hold the door open for a younger woman when she is capable of not only holding for herself but for him as well? And anyone can and should hold a door for a person who is obviously disabled. But if you're both able-bodies, why not simply take turns. That way, you don't have to hold the door and then rush ahead to hold the next one.

The modeling/finishing school I went to said the man opens the first door, and if there is a second door the woman opens it.
In your case you go in front of her and push the door open then hold it open for her.
I've seen men stand to the side of a woman and struggle to push the door open straining their arm while she goes in first. But that looks awkward.

Walk through and hold the door open from the inside.
If your lucky before you know it 3 or 4 people have also walked in thinking your a door man and slip a $10 note into your pocket...not bad!

If the door is difficult to open, that is it requires a hard shove, it is okay to go through first and hold the door from the other side.

Yes, go through first and hold it open for her. I always think it's ridiculous when people try to lean forward to stick their arm out and hold it open to let you go first. It just ends up being awkward because what they really accomplish is getting in your way so you don't really have room to go through the door without tripping over them.
Besides, even if you technically go first to hold the door open, you're still really letting her go first because she will end up ahead of you after she goes through.

You push the door open, enter inside, and then hold the door from behind alliowing the other person to enter.

you push the person infront of you, stand against the door facing the people spread your arms and legs and form a T formation, and yell "NO ONE, I SAY NOONE WILL OPEN THIS DOOR!", then raise your hands and mumble something weird and tell the folks that you have cast a magical spell on them and if they move they all shall turn into frogs and will be sent to a French restaurant where they will have their legs torn apart, then chain the door and put a sign that says "The Forbidden Door" and jump off a window yelling "SHES GONNA BLOW"

My husband goes through first, holds it for me. Standing outside and pushing the door in to allow the lady to enter is awkward. I would let her know, "I'll get that for you" so she knows what you're up to.
