You I-94 will be stamped "Single Entry" or "Multiple Entry".

If it was "single entry", you don't need to do anything. As soon as you exit the United States, it is voided and you cannot get back into the country with it (basically, you would have to apply for a new one).

If it was "multiple entry", and you are travelling by air, it will be stamped with your date of exit (and reentry, should you wish to return) every time you exit/enter, provided you haven't exceeded the duration for which it was issued. If you are travelling by other means (automobile, rail, or sea), you probably won't get anything on the way out, but you WILL get it stamped and checked on the way back into the United States ... provided you're back before it expires.

If you are leaving for good, then there's nothing you need to do from your end. The visa will expire either on your departure from US soil (single entry), or by 'timing out' (multiple entry). Of course, if you were a conscientious traveler, you'd be sure to return it at the US point of exit - or at least stop in and ask them if you should.

You don't have to surrender anything unless you know "FOR SURE" that your not coming back to the US.
