OK. well if you want to get a very high ponytail you have to brush it all the way. No tangles. Brush from the top and go down. Then flip your hair over and brush the underside. Get your hairtie ready and stay flipped over. Start pulling with force upward. from the front and back.
For a normal ponytail behind your head...
Use your brush to get all the tangles out. Then keep using your brush and pull back. this will get the bumps out. then pull your hair tight and tie it back.
Don't put it up when it's wet...it will rip your hair out and cause breakage. Plus it gets frizzier when you do it wet. Dry it first or let it dry naturally before you do anything to it.

my friend used to be obsessed with doing perfect ponytails...this is how she did it...
...gather all your hair up by flipping your head over.
then brush all of it so there are no bumps. keep you hand ( the one holding all your hair) loose (as in a littlt far from your scalp).
then spray hairspray all around your head
comb your hair to get rid of any more bumps.

You need good rubber bands
also you need good hair moistureizers
bend over then brush your hair up from the back
then brush it from the front then put your rubber band on it
then tighten it

I usually lay on my bed with my head hanging off the edge and brush my hair and pull it into a ponytail. That way, gravity pulls it into place without having to fight to pull it all up.

get a bobble ready on your wrist, bend over so all of your hair drops down the gather it all up and stand straight again. then use your hand to gather up all your hair. then tie a bobble on. (if it doesnt work wear a headband while doing it)
after tying my hair up, i always wiggle the bobble at the top of the ponytail up and down so it droops down a little then i scruff my hair a bit and pull out a few strands of hair near my face!

its easier to make smooth if the hair is a little damp... or use spray or gel...
but your hair is also weaker like this so don't secure the ponytail too tight.. the hair will break and then you will have problems with smoothness...
anyway perfect ponytails can be boring... its nice to have wee wisps... looks soft and natural.... very attractive...

My female friends can't do a perfect ponytail so they go to the salon, and they pay very little and they do it perfectly. You can try watching carefully, and you might learn how to do it perfect. =]

try putting it up while your hair is wet or damp. flip your head upside down, use a good wide toothed comb. keep combing while holding your hair with the other hand till all the lumps are out.
hope this helps
