NO, NO, NO...cookies have enough grease inside the dough. Adding more to the cookie sheet will make them spread out flat.
And adding flour is just dumb as it will only burn where the dough doesn't cover...

Line them with parchment paper. The only way to go.
No way, no how!
And you should always bake them using 4 cookies sheets, 2 in the oven and 2 out loaded up with the next batch. Why? Because that way you'll always be loading the next batch onto cool cookies sheets. Never put cookie dough on hot or warm cookies sheets.
And last, but not least, never bake your cookies in more than a 325 degree oven, unless the recipe calls for lower temp. If it says 350, don't do it, go with 325. That is straight from Mrs. Fields.

This sounds bad, but it depends. I used to grease cheap Ecko sheets from Wal-Mart, and couldn't scrub it off. I got some Analon pans and they said to NOT use spray. And they clean up really well. A good investment for $10 a pan from Target.

It isnt neccessary to grease the pan for cookies. If you must though, just spray Pam on the sheet. You can also use parchment on the sheet to set your cookies on when you bake them.

Depends on the recipes. If you don't have an idea - grease it up anyways - it makes the cookies slip off the pan when they are done.

Yes. That way, they don't stick to the pan. It's faster if you just spray the pan w/non-stick cooking spray though.

It depends on the recipe you're using. If it contains a lot of fat, then no, you don't grease the sheets. If there's less fat, then grease.
