Just a mattress (I've never even seen a box spring that's crib size), but you'll probably want to get a large piece of cardboard or a thin piece of particle board to put under it so that it doesn't get indentations in it from the metal grid it sits on.

The crib skirt goes under the mattress. Your crib will either come with a board of a set of springs that hold the mattress. You dont use a box spring mattress with a crib.
