It is usually intended as insult - taking the Enlightenment idea that the Middle Ages were unenlightened, ignorant time. Therefore, to have medieval attitude is to be fearful and ignorant. It can also be used to mean "brutal", as in "medieval torture" or "to get medieval on" someone.
Ironically, this understanding of what the middle ages were like is itself outmoded. Most historians today reject the idea of a "dark ages" as a pejorative invented by Enlightenement philosophes to make themselves look better. A lot of progress was made during the Middle Ages which is only just beginning to get its due.

I'm not sure about your area, but to me, the use of 16th century english that is often missunderstood, (bible english acd to KJ)

It means to be archaic, like someone who believes that disease is caused by sin, that women are not to be trusted, concepts that were popular during the middle age.
