If I have surgery on it, will I look different a lot

braces on your teeth would help and not change your appearance radically. plastic surgery would do the same faster. people would wonder what was different, but most would not know what it was.

Yes, you will look different, not drastically though.
Before you have surgery, make sure you get a second opinion as it can be fixed other ways at times.
Also, sometimes it can't really be fixed by braces and surgery is necessary.
If it's solely for cosmetic purposes, there might be even more options.
If you don't want to look any different, and it won't affect your jaw in the long run which your dentist should be able to tell you, leave it the way it is.
If you are looking at surgery, be prepared for long healing times including your jaw being wired shut for up to two weeks.
This all happened to me. One dentist told me surgery, i got a second opinion and they disagreed. My issue won't bother me in the long run, so I'm just going with Invisalign and porcelain veneers, but that won't work for everyone, depending on how significant the underbite.
