You don't say how old you are or how much you have to spend. If you dont have a lot get flowers or chocolate, they always go down well, if you have more Royal Doulton or a similar ceramics company do figurines that are named, you could maybe get one that is your mums name. Cook a meal for your mum or take her out for dinner, or invite your relatives and her close friends over for a surprise party. You know your mum best and will know what shed enjoy the most

Think back to a time something special happened that gives you the warm fuzzies between you and mom. write it down and enclose it in you birthday card . Give it to her as the two of you are alone . I assume #40 means there will be several people involved .Leave the black balloons and over the hill jokes to someone else . If its just the two of you go see a movie together and to a place she remembers from the time she was your age. and encourage her to tell you about herself and that time in her life.You might do that even a day after whatever big party deal happens.

How about a nice locket,( or a pretty picture frame) with both your pictures inside and a card with your heartfelt words on what she means to you and how much you love her!! Take it from another 40 plus year old mother, it's always nice to hear that from your children!!!

A 3 day Las Vegas trip...you can get room now for $19 a night...it's off season. My daughter did it for me...it was great!

get a whole bunch of family pictures and make a scrap book mothers love that kinda stuff :)
