For a 3 year old to be having diarrhea and itching in that area, I would take her to the local medi-center and have her checked. Diarrhea in itself can cause itching. Other causes could be pinworms, the lotion you are using (use A & D Ointment), not washing properly, washing too much, etc.) If the diarrhea continues, medical help should be seeked. You don't want her to get dehydrated. Offer pedialyt or a sports drink or other oral electrolyte solution. Lots of fluids and rest.

Sounds like a yeast infection. Get some antifungal cream like athletes foot cream, jock itch cream or vaginal yeast cream and start applying that a couple of times a day after washing well. Also get the diarrhea under control with some Immodium aka loperimide.

try having her sit in a luke warm bath with some baking soda for about 20 minutes, then allow her bottom to air dry. No soaps to her irritated bottom.

hope this helps

Bring her to see a doctor and get a proper diagnosis. Do not self medicate or self treat until you know what is the diagnosis.

Do you have any Benadryl?
You could try putting her in a warm bath with cornstarch.
