First notify any and all affected persons in the area of your intentions to lock out and work on machine.

Gather all needed PPE -safety glasses, gloves, etc. whatever the situation and company standards require.

Lock out/tag out all power sources.
Electric, pneumatic, hydraulic.etc.
Most companies will have a set standard you must adhear to when locking out and working on machines.
Knowing all your companies exact rules are part of your job.

Release any stored energy in the machine.

Try to start the machine to double check that every thing is incapable of accidential starting.

Make sure the work area is clear of any obstructions, clutter, oil/water spills or any other potential hazards, and remove them as needed.
Also make a quick mental note of any other potential hazards in the area(Line of fire hazards), such as a protruding pipe or a sharp edge which you could potentially injure yourself on during the course of the repair and take any neccessary precautions such as covering them with a shop towel or appropriate padding.

Remove all necessary covers/guarding.
Make an initial inspection or machine, look for any abnormal wear to the belt, pulleys, gears, etc. check the area below for any signs of grinding ie metal particles or rubber "dust" .

Check for loose conduits and wires.
Look for any loose bolts and or mounting brackets etc.

Inspect beaings for any signs of slop. The belt or gears may be worn , but is could just be a sign of underlying issues such as bad bearings, worn shafts etc. Which in turn could be signs of a faulty lubrication system in the machine.

Check the control circuit of the drive motor, you may have worn contacts in the motor starter or loose connections, causing the motor to run hot and over heat it's bearings.

Try not to be fooled by symptoms, look deeper into the machine and try to find the root cause of the failure.

Once the actual problem has been diagnosed, make repairs as necessary.

Clean up any excessive grease, oil etc, left from repair.
Make sure no tools etc.are left inside the machine and then re install all covers/ guarding.
Remove any padding or towels you had used to cover any potential line of fire hazards.

Wipe down the machine of any and all greasy hand prints you may have left, clean up all trash and other left overs from the repair.
This isn't actually necessary but it is best if you expect any future cooperation from your co workers who run the machines.
They are YOUR CUSTOMERS, and leaving a clean work area is all part of good customer relations, and shows that you respect them and are concerned with doing a good job.

Remove your locks and tags.
First Notify any affected persons in the area that you are about to re energize the machine and to take proper precautions.

Power the machine back up.
Have the operator (if you don't know how to operate machine) run the machine through it's paces to verify proper operation, while listening for any abnormal noises, checking for leaks, smoke etc.

And ta- da!
You've sucessfully created another happy customer.
Time to move along to the next job at hand.

Elf has covered most of it,

i'd change the following the following:

step 1: carry out a risk assesment before starting any work (including isolations)

risk assesment should include:
slips trips and falls (is area of work free of any hazards)
working from heights (do you have adequate access to area)
ventilation (are you going to be able to breathe where you are working)
safe isolation (isolating is part of the job, dont neglet to include the isolation point as part of your risk assessment)

otherwise i'd agree with elf 100%

(note: remove tie if worn) ((not that any of us engineers voluntarily wear them))

hi...also 'get'... 'isolation warning' paperwork from the company concerned, and this should be signed by a manager, also the 'time and location' you started the work, 'what' equipment you intend working on, etc etc...and then get it signed off on completion, and keep a copy for 'your' company, and for 'your' own record.

plus as the other guy said...put 'power isolation' and 'warning' signs up...at the power distribution area for the equipment you are working on...and at the 'place' you are working...ie. the door into the room where that piece of equipment is situated.
