I'd report it to the bus company and also the police. The Bus Company may take a dim view of it, have a word and do nothing or they may just ignore your letter.
If the police are involved, they'll ask the Bus Company what's been done, they can check the logs for both the driver and his mobile records and take the appropriate action.
He may have been lucky when you were on his bus but how soon before his luck runs out and someone dies.
Don't feel guilty that a man may lose his job and his family and his house, etc., he's broken the law. He knows the rules and he still broke them.
Put it this way, he put YOUR life in danger through his selfish behaviour - how does that make YOU feel?

You can report him if you have the Bus # on the bus and the route your road the bus on. Report the Driver anyway.
The driver should only be talking to the dispatcher or passngers for information of bus routes fairs or if a little lost gor guidence.
He put your life and others at risk .
If you live in Chicago it is against the law to talk on the cell phone while driving even if you have the headset. People still do it in Chicago with the headset.

If a bus driver uses his Mobile while driving he can be sack as he is breaking the company's rules .. so yes report him as long as you have proof

Report him to the company, and let the police know. The company should be able to trace who he is from the bus journey time and/or through the numbers on your ticket. IMO that should be a dismissable offence.

Yes, immediately, if you have any concerns about safe motor vehicle operation. Get as many other customers as are with you focused on the problem before they get off. Use your cellular to notify Transit Management of the problem and what stop you would would like to have a supervisor waiting to recieve complaint. Or, tell operator you want to see the immediate supervisor soon as possible. Wait a week and you get blown off.

yes please report him cus what happens if hes to busy on the phone and forgets to stop at a red light? or forgets to stop and pick some one up? some ppl really count of the bus for there jobs
just call them or mail them saying something
Do you know the bus drives name? if not thats ok
just rember what bus you was on and about what time it was

By all means! If you have pertinant info, such as badge, bus, route numbers and time of travel, you should call it in to the transit company. Most transit companies have a customer service number you call, to report such violations.
