Absolutely, positively stay away from the heartworm injection, and opt for monthly preventatives. That injection killed many dogs the first time it was on the market, and injured many more. It's not safe to give a dog 6 or 12 months worth of what is basically a pesticide at once. My sister's vet convinced her to give it to her dog, after I had already advised her against it, and she spent $5000 on medical treatment for the dog's damaged liver, and is still spending money every year on related testing and treatment. It's too much for the dog's organs, and there is no way I would do it. For information, Google for damages caused by the "proheart 6" shot.

If the dog is already infected, treatment can be very expensive to save the dog`s life, If not infected, preventive medication runs around $60, plus blood work to rule out infestation. Heartworm can be fatal, especially to puppies. So use care and caution.

Depends on where you live and what you view as a medium sized dog.
I'm in Australia and have a 22 kg dog. I bought a pack of 6 Sentinel Spectrum tablets for her today at a cost of AUD $92.30. They are given monthly and cover heartworm, roundworm,whipworm,hookworm, and tapeworm. They also prevent and control fleas long-term.
My dog has no trouble taking them broken up with her food and no sign of any nasties to date. She's been on them for years!
