You call everyone you care about and tell them what a wonderful thing (specific) that they did for you personally that made u a better person.
call your family and tell them you love them.
Then call a travel agent, and use your visa card and book the most EXOTIC VACATION OF YOUR DREAMS, AND GO WITH YOUR LOVER, OR BEST FRIEND-- and live while you are dying!
oh-- and don't forget to watch the sunset and sunrise every day you have on this earth.
LIVE EACH DAY, AS IF IT WERE YOUR LAST!

You try to live your remaining days of life to the fullest. Spend as much time with your family as possible. Tell them right away and don't worry about them being devastated because they'll be even more devastated if you suddenly "drop dead".
