My soon to be ex husband had to sign one of those (we are in Georgia). At first I thought that was a form to sign because he is homeless, but I read on it and it just means that taxpayers have to pay for his court appointed attorney.

I can't imagine it would mean anything other than signing off saying you're unable to afford an attorney, fine, penalty, etc.

That you swear and affirm that you are unable financially to provide for adequate counsel for your defense. It is a device used to justify spending tax payer dollars to defend someone who committed a crime (or has been accused of committing a crime).
