Cohesive Silicone Gel. It looks more natural and doesn't leak if it ruptures. HTH : )

I think that saline is better.
If an implant should leak, your body absorbs the saline without a reaction to it because it is a natural solution that is in your body - saline is a salt water combination that is used in many things such as contac lens solution & doctors use it during surgeries etc. Silicone is not a "natural" ingredient which can cause the body to have a reaction to it if an implant should rupture.

From a guy's perspective?
I'd say natural.
It is easy to move fat from an unwanted area and bring it to where you need it. And you will never have to worry about saline popping (it is the worst alternative) or silicone infections.

saline is safer but feel like water baloons.
silicone can be dangerous even tho 'they' won't admit it, but they feel natural.

silicone is better but it is more dangerous. so it's really a matter of personal opinion.
