The standard merchant agreement for Visa/Mastercard specifically forbids adding the cost of a credit transaction onto the customer's total charges due, but it could in theory be modified to allow that to take place. If a merchant operates under the standard contract with Visa/ Mastercard and they pass on the cost of the credit card transaction as a seperate charge then they are in breach of contract with the organization that provides their service. If you see a charge on your sales receipt that clearly identifies a charge for a credit transaction, call the merchants service provider and inform them of the charge. Merchants may, however, pass on the cost to all of their customers and provide a discount to cash customers. There is nothing that you could do to the store except report them to their service provider for breach of contract. But, remember that it is possible that the merchant has a non-standard contract and you have to call the company that processes the credit cards for the merchant. And, this only holds in the USA, I'm not sure about the applicability of my answer in other countries.

yes, i believe they can charge you pretty much whatever they want as long as they notify you that you will be charged, and then have the choice to go forward with the transaction or not. companies incur processing fees and other costs in order to be able to accept credit cards as payment. some establishments will compensate for that by charging a fee added onto your transaction. even though its frustrating, it is a legitimate business practice

I think so. Especially bad are foreign country transaction fees--I got charged like 50 dollars to buy a plane ticket in London!
