I wanted to get my mom a manicure since her hands are very badly damaged over the years, but that may be a little expensive. My mom herself is going through a very rough phase, and it's very tough for her.
The thing she wanted the most would be to spend time with me. Maybe take her on a walk to the park, it'd get her mind off things. If she doesn't like walks, maybe cook with her, throw a small party with the food you and your mom made, just you, your mom, and some of her friends if she has some to invite.
If this is complicated, you could always help her chores and plant something outside her window, like a small bright flower. Be careful about the flower type, like it's all year.
Or watch a movie with her and go the mall with her.
Maybe sit down with her on the couch and watch tv together. The simplest things seem like nothing, but they're very nice to have or give.

Why don't you and your Mom go to one of those walk-in manicure/pedicure places and get your hands and feet done (you should wear sandals (maybe too cold), get a bite of lunch and maybe take in a movie. Spending time with her will make her day. If that's a little too much--might cost $150--have someone take a nice digital picture of you and put it in a frame. She will love anything that you do for or with her. Does she have a favorite chef on the Food Network. Maybe a Paula Dean(sp) cookbook would be something she'd like. Tell your Mom we said Happy B'day!

My mom just hit 46 and I got her a necklace from my daughter she loved it. I would suggest a rose bush one she can keep inside until its time to put outside something she has a hobby for, or a trip to the spa, take her to favorite restaurant something she will enjoy. Maybe even just some alone time out with you, or a good book.

look around the kitchen to see what she doesn't have and you think she may need - a bread machine, blender, cake mixer, etc. Maybe all she'd like is a bunch of flowers, and/or a nice blouse - moms always treasure whatever present their children give

sounds like your Mum could do with a bit of pampering..check with your local Beauty Clinic..they should offer a full range of vouchers for all sorts of treatments from facials to manicures...there should be something that will suit your budget and most Mums wouldnt think of buying that for themselves
I hope your Mum has a lovely Birthday no matter what you decide to do..just spending quality time with her will mean a lot to her

I have four children. My favorite birthday present was: Each one hugged me, told me they loved me and told me thank you for being my mom! We then had ice cream Sundays because they are my favorite. A gift from the heart is always a hit with a mom!

We always give my mom a "care package". We put in a couple cans of soup, single serving size popcorn, cookies, crackers, chocolate, and some bath beads in a nice basket. She always looks forward to it.
